- Organisation details, including name, address, telephone number and email address.
- Name and address of the people in charge of the organisation.
- Name of the organisation's bank.
- The percentage of the members/people benefitting from your activities that are Calderdale residents.
- The fees the organisation charges for activities.
- Registered charity number (if the organisation is a registered charity).
- Community amateur sporting club number (if the organisation is a community amateur sporting club).
- Limited company number (if the organisation is a limited company).
- The amount of cash in unrestricted reserves the organisation has.
- The organisation's unrestricted income last year.
- The number of staff employed to run the bar (if the organisation runs a bar).
- The gross income from the bar last year (if the organisation runs a bar).

- Any other information about your organisation that you think we should consider when we assess your application.
